CPHFH Recertification

Following successful completion of the CPHFH examination, the certificant is required to maintain certification by fulfilling continuing education (CE) requirements or through successful retest.

​​​​​​​​​​​​​​​​​The Certified Professional in Human Factors in Health Care (CPHFH) program certifies health care professionals who meet the competency requirements in the areas of human factors engineering, and who demonstrate the ability to apply this knowledge to effectively plan and implement human factors initiatives.

Following successful completion of the CPHFH examination, the certified professional is required to maintain certification by fulfilling continuing education (CE) requirements, which are reviewed and established by the Certification Board for Professionals in Patient Safety (CBPPS).

Recertification cycle: 3 years

Two paths to recertification are available, as described below. Please submit your application to certification@ihi.org 

Recertification Path I 

  • Includes obtaining and maintaining documentation of 45 CE hours over the recertification cycle, submitting your completed and supporting documents, and payment of a recertification fee.
  • Accepted types of CE:
    • All CE that follows the CPHFH content outline is accepted.
    • CE options include: educational programs, self-study, CPHFH item writing, academic course work, and more.
  • There are many ways to obtain credits toward your CPHFH recertification. The CBPPS has pre-approved the programs listed on the CPHFH Continuing Education Credits web page
     

Note: Before filling in the CPHFH Recertification Application, save it to your desktop. Otherwise, anything you type will not be saved.

Recertification Path II

  • Requires sitting for and passing the CPHFH Examination within one year prior to expiration date.
  • Submit first page of application with certificant information and PSI passing score report.
