This article describes the approach led by 100 Million Healthier Lives, a collaboration of partners across sectors convened by Â鶹ӳ», to support the measurement of health and well-being at local, state, and national levels. The work resulted in development of the , which aim to meet the needs of stakeholders that are struggling to improve population health and to address social determinants and equity across sectors.
